Camp Forms

Registering for Camp or a Family Trip? Simply SIGN UP TODAY to register online. You do not need to fill out any of these forms.

You will need Adobe Acrobat Reader installed on your computer in order to view or print PDF files. It is available as a free download on the Adobe Website.

How do I know which forms I need?

After you register, we will email you the basic forms you need. Please refer to Day Camp, Sleep-Away Camp, Family Trips, School Programs or our all forms sections below if you need additional forms, or would like a copy for your records.

Day Camps
Online registration:
If you registered online then you electronically signed our Camp Risk Acknowledgement & Release Form, as well as a Day Camp Health Form and therefore do not need to bring them to camp. During the online registration process you either selected to bring a copy of the Colorado Immunization form or you selected to sign the personal exemption electronically. If you choose to bring a copy, please do so on the first day of camp.

Phone registration:
If you singed up via phone, then we will email you a filled out Camp Risk Acknowledgement & Release Form, as well as a Day Camp Health Form to sign and bring to camp on the first day. The only other thing you need to bring is a Colorado Immunization Form. HOWEVER, if you cannot locate your current CO Immunization form, then you can simply sign our “Personal Exemption” form the morning of registration and you will be all set.

Forms needed for medical purposes:
If your camper carries Epi, you will need to fill out the Severe Allergy Health Care Plan form. If your camper uses an Inhaler, you will need the Respiratory Health Care Plan form. If medications must be administered at camp, you will need a Medication Administration Permission for Day Camp form. If you don’t have current medical insurance, you can apply for a short term policy.

Sleep-Away Camps:

Online Registration:
You most likely filled out the Overnight Camp Health History form information during that process. We will email you a filled out Camp Risk Acknowledgement & Release Form, Camp Health Form, as well as any other required forms for third party providers (such as horseback riding release). Some of these forms require a Physician signature. 

Forms needed for medical purposes:
If your camper carries Epi, you will need to fill out the Severe Allergy Health Care Plan form. If your camper uses an Inhaler, you will need the Respiratory Health Care Plan form. If you don’t have current medical insurance, you can apply for a short term policy. All forms for Overnight Camps must be returned to the Avid4 Adventure office no later than two weeks before the start of the trip.

Family Trips:
All forms will be provided to you via email before the trip.

School Programs:
The On-Camps School Program Risk Acknowledgement and Release form or Off-Camps School Program Risk Acknowledgement and Release form must be turned into your school.
