Forms
- Day Camps- If you registered online and filled out the health history information during that process, then we will email you a filled out Camp Risk Acknowledgement & Release Form as well as a Day Camp Health Form to sign and bring to camp on the first day. The only other thing you need to bring is a Colorado Immunization Form. If your camper carries Epi or an Inhaler, you will also need the relevant Health Care Plan form (see below). If medications must be administered at camp, you will need a Medication Administration Permission for Day Camp form (see below). If you don't have current medical insurance, there is a link below to apply for a short term policy.
- Overnight Camps- If you registered online and filled out the health history information during that process, then we will email you a filled out Camp Risk Acknowledgement & Release Form, Camp Health Form, as well as any other required forms for third party providers (such as horseback riding release). Some of these forms require a Physician signature. If your camper carries Epi or an Inhaler, you will also need the relevant Health Care Plan form (see below). If you don't have current medical insurance, there is a link below to apply for a short term policy. All forms for Overnight Camps must be returned to the Avid4 Adventure office no later than two weeks before the start of the trip.
- Family Trips- All forms will be provided to you via email before the trip.
- School Programs- School Program Risk Acknowledgement and Release form must be turned into your school.
